STAMA Integrated Health Solutions provides Case Management Services to support individuals in accessing, coordinating, and maintaining the services they need to live healthy, stable, and meaningful lives in the community.

Our case managers work collaboratively with individuals, families, providers, and care coordinators to assess needs, develop and monitor person-centered plans, coordinate services, and connect individuals to community and natural supports. Services are designed to promote continuity of care, informed decision-making, and progress toward personal goals.
